Where is the Stone Bench?
We can reason that the small ruins are fairly close to the Black Rock. Richard was extremely weak when the Man in Black rescued him, so he probably would’ve taken him to a nearby location so that Richard would not be weakened even further by a long journey.

Jacob's Cabin
Jacob’s cabin doesn’t appear on the map for a couple reasons. First, it appears to be capable of moving, as well as disappearing and reappearing. Hurley first encountered it in a different area than Locke and Ben. Eventually, we see a map of the cabin:

I was watching the episode, “Through the Looking Glass” last night, and noticed that when Ben and Alex were hiking to intercept the survivors, who were on their way to the radio tower, Ben tells Alex that they need to head north to reach the tower. A note on vehicles
On other Island maps I’ve seen, some authors have added markers for various vehicles on The Island, like the DHARMA van or Desmond’s sailboat. The reason I’m not adding them is that they are no longer in the locations where they were discovered. They are fully-functional vehicles that are capable of moving around The Island, and therefore can’t be marked on a map.

Where is the Orchid?
We know that there is a well very close to the site of the Orchid. During the time of the DHARMA Initiative, the well is filled with earth, but in the past, it provided a way to access the frozen wheel chamber. The problem with Pala Ferry
When Pala Ferry appears on the show, the water surrounding it is very calm. This suggests that its location is in protected water, away from the crashing waves of the ocean. Well, here we are. We finally have to deal with an Island location that seems to move around as much as Jacob’s cabin: Hydra Island.
The producers confirmed that they always intended Hydra Island to be one of the smaller islands on Rousseau’s map. According to the blast door map, it should therefore be located at position A.
